We protect your healthcare information in compliance with federal and state requirements.

To receive a copy of your medical record or to authorize San Ramon Regional Medical Center to release your medical information to someone else, you need to send your request in writing. Please download the Patient Release of Information form below.

Our Health Information Management Department is open from 8:00 a.m. to 4:00 p.m. Monday through Friday.

Requests may also be emailed to [email protected]. Please note that there may be a fee for creating a copy of your records.

You can also access your records online on the Patient Portal.

Accessing Radiology and Cardiology Cath Discs

Requests for radiology and cardiology disks are processed by our radiology department.

The number to call for Radiology and Cardiology disks is 925-275-8273.They are open from 7:00am to 5pm Monday thru Friday and 8:00am to 3:00pm Saturday and Sunday.
