You can register and access your records online on the Patient Portal.

We protect your healthcare information in compliance with federal and state requirements. If you would like a hard copy of your medical records, please fill out an electronic request for medical records using the link below.

There may be a charge for creating a copy of your records.

Accessing Radiology and Cardiology Discs

Please note that requests for radiology and cardiology discs are fulfilled in our radiology department.

After filling out a patient release of information form (link above), you will be contacted when your images are ready to pick up.
