Preparing for Hospitalization 
Thank you for entrusting your health care to San Ramon Regional Medical Center. To help you prepare for your hospital stay, we  provide a list below of items you should and should not bring to the hosptial. We also have included information on admitting hours, consent for treatment and advance directives.

What to Bring
  • Your health insurance card and your spouse's, if applicable; all insurance forms and paperwork; e.g., pre-authorizations.
  • Valid driver's license, state identification card or passport.
  • Advance directive documents if you have them.
  • A list of medications you are taking including dosages and directions.
  • Any lab test results and pertinent X-rays not taken at San Ramon Regional Medical Center.
  • Personal hygiene items.
  • Clothing for your stay in the hospital and for your discharge.
  • Your eyeglasses, hearing aid or dentures.
  • Reading and writing materials and other personal items you'll want during your stay.
  • If desired, cash to pay for newspapers, gift shop articles, and other items. Not more than $20.00 in cash is recommended.
  • Credit card number and expiration date. NOT the credit card itself.
  • Calling card number.
If you are going to the Critical Care Unit (ICU/CCU), limit personal items to those that are absolutely necessary, such as a robe, eyeglasses, dentures, hearing aid or other prosthetics. We will provide you with slippers and grooming items.

  • Jewelry
  • Valuables
  • More than $20.00 in cash

San Ramon Regional Medical Center cannot take responsibility for loss of these items. Should items of value be brought to the hospital, we recommend they be deposited in the safe in our Admitting department. Notify Admitting or Nursing Staff of the items to be depostied and be sure to ask for a receipt, or send the items home with a family member.
Admitting Desk Hours: 6 a.m. - 6 p.m., Monday - Friday

During other hours, please register in the Emergency Department. 

You may pre-register for your hospital stay or outpatient service by calling the Admitting Department at 925-275-8257.

Consent for Treatment

You will be asked to sign a consent-for-treatment form giving us permission to treat you.

Advance Directive

The Advance Directive for Healthcare allows you to designate someone to speak for you if you become unable to make decisions about your care. On admission, adult patients are asked if they have a current Advanced Directive. If so, the directive is placed in the patient's medical record. If not, an informational pamphlet and form are available. Request for the pamphlet and form may be made through the Admitting department at 925.275.8231. You are not required to have an Advance Directive in order to receive care.
